VEP Supervisor Feedback

The Volunteer Performance Feedback Form should be completed as often as you like but consistently.  It can be completed online, or you can print a NYEP Volunteer Performance Feedback Form form and have the youth volunteer return it to NYEP staff.  Thank you for your commitment to fully support youth as they learn the skills and knowledge to be successful in life.
